Ballarat Police senior constable Ben Hay said the man was the pillion passenger on a motorcycle travelling west on Mair Street when it collided with two parked cars near the Lydiard Street intersection at 6.10pm.
He sustained possible multiple internal and head injuries and was treated by paramedics at the scene before being taken to Ballarat Base Hospital.
The rider, a man in his 30s, was not injured and was assisting police with their inquiries.
“Investigations are still ongoing as to possible speed and alcohol,” Senior Constable Hay said. “He appears to have lost control and collided with parked cars.”
The crash occured in front of Jacksons and Co restaurant, bringing a small crowd of diners onto the street.
The west-bound lane of Mair Street was strewn with glass and blocked off to traffic while police questioned witnesses.
Senior Constable Hay said it was too early to say if the traffic lights were red or green at the time and urged any witnesses to come forward.
Ballarat Base Hospital spokesperson Daniel Anderson said the man remained in a serious condition and would be transferred to a hospital in Melbourne overnight.
Witnesses are encouraged to contact Ballarat Police Station on 5336 6000.
