Police say the woman, 78, was hit around 9.30am at the intersection of Victoria and Humffray streets, Bakery Hill.
The driver of the maroon-coloured Toyota sedan stopped to render assistance.
Two ambulances were on scene Tuesday morning and the woman was transported to Ballarat Health Services Base Hospital with lacerations to her head.
Two occupants of the vehicle were uninjured.
Police blocked the roundabout for 20 minutes until the scene was cleared.
