The truck was believed to have been travelling south along High Street, Preston, when it turned onto Rosebury Avenue, striking the woman shortly after midday, a police spokeswoman said.
Emergency services and police were still on the scene at 2pm, with High Street closed between Murray Road and Cramer Street.
The woman, believed to be aged in her 70s, was treated at the scene by paramedics for serious leg and chest injuries before being taken to the Royal Melbourne Hospital, an Ambulance Victoria spokeswoman said.
It is understood she may have been trapped momentarily after being struck.
A man is currently assisting police with the investigation.
A Royal Melbourne Hospital Spokeswoman said the elderly woman was in a "serious condition".
