L.U. Simon Builders construction director Jim Moschoyiannis and Con Nicolas, the fire safety engineer who worked on the Lacrosse building, have been referred to the Building Practitioners Board for disciplinary action.
The Victorian Building Authority will allege the pair failed to carry out their work in a competent manner and to a professional standard.
They are not the first building practitioners to face disciplinary action as a result of the blaze, which was fuelled by highly combustible, non-compliant cladding installed on the outside of the building.
The registered building surveyor, Anastasios Galanos of the Gardner Group, is also facing claims he did not carry out his work to a professional standard, while the project architect has been referred to the Architects Registration Board.
It is understood the tower was designed by high-profile Melbourne firm Elenberg Fraser.
On November 2014, a blaze sparked by a cigarette on the balcony of an apartment created the fire that ripped up the facade of the Lacrosse building in a matter of minutes.
The owners of the apartments in the 23-storey tower are now facing a multi-million dollar bill to remove the cladding, as builders L.U. Simon have so far refused to rectify the problem.
