The woman's Mazda sedan was turning into Pound Road from Narre Warren-Cranbourne Road in Narre Warren South about 10.30pm when the stolen Ford hit the rear of her car, a police spokeswoman said.
The Ford driver jumped out and fled the scene on foot.
Both vehicles were badly damaged and paramedics assessed four people.
The woman, aged in her 40s, suffered neck and chest injuries and was taken to Dandenong Hospital in a stable condition.
A primary school-aged child suffered bruising and was also taken to the hospital.
The Mazda driver, a 45-year-old Narre Warren South man, and another male passenger were not injured.
Police are still searching for the driver of the stolen Ford.
This is the second incident of its kind in less than a week.
Police are also searching for the driver of a stolen car that crashed into a vehicle in Dandenong on August 28.
A police spokeswoman said a white Holden Commodore with a stolen number plate crashed into a silver Toyota sedan at the intersection of Axel and Stud roads about 8pm.
Police believe the Holden drove 100 metres north of the crash site before a number of people got out of the car and fled the scene.
A woman and three children were in the Toyota at the time of the crash.
The woman and one of the children suffered injuries and were taken to the Royal Melbourne Hospital and the Royal Children's Hospital for treatment.
The other two children were not injured.
The spokeswoman said police were searching for the driver and occupants of the stolen Holden.
