Matthew Stephenson was last seen on a bus heading towards Wyndham Vale railway station about 6.30pm on Sunday night.
Matthew had got onto the bus at a stop in Shaws Road, Werribee.
Police hold concerns for Matthew's welfare due to his age and because his disappearance is out of character.
They have released an image of him in the hope that someone may have information on his whereabouts.
Matthew, who lives in Wyndham Vale, is about 140cm tall, of skinny build, with black hair.
He was last seen wearing a grey North Face jacket with blue sleeves, a red Nike T-shirt, yellow Werribee Devils basketball shorts and black shoes.
