Worksafe has blamed bottle-necks at exits and unstable ground for the stampede at Falls Festival, with one doctor saying he was amazed no one was killed in the crush.

More than 60 people were injured and 19 hospitalised during a human crush at the Grand Theatre stage of the Lorne Falls Festival as fans rushing to see a headline act triggered a stampede on Friday night.

Thirteen women and six men were taken to hospital overnight with lower leg fractures, pelvic fractures and cuts and bruises.

Creative Industries Minister Martin Foley on Saturday described the stampede as "unacceptable" and "very dangerous" and wished all those who were injured a speedy recovery."Worksafe has investigated and worked with Falls management and has recorded that they've made some immediate requirements for change to the facility to make sure that there is not a repeat event."Mr Foley said that the festival's organisers had removed all bottleneck points with "the venue now, in Worksafe's view, safe."A government spokeswoman said Worksafe had also recommended that the festival's managers strengthen fencing at the front of the venue and place treading on the ground to prevent people falling over. She said that Victoria Police had also been working with management on safety issues at the venue.A Worksafe spokeswoman said inspectors had been at the festival today: "Worksafe is aware of the incident and is making inquiries."Mr Foley said he was confident that the festival could continue in a "safe and controlled environment".

Festival organisers have said the Grand Theatre met building regulations and guidelines, and that normal programming would resume on Saturday.

A spokeswoman for the festival said all exits at the venue were open, with the only people held inside the tent "those who were in a direct line of those who were injured, so they could be attended to."

She said the festival had adhered to the venue's maximum capacity of 16,500 people.The Australian Medical Association's president Dr Michael Gannon described footage of crowds at the festival as "frightening". He said he was amazed people did not suffer more serious injuries in the stampede, and that no one was killed.

"It is quite frightening vision ...just the sheer volume of people and reports of people trying to leave through a narrow exit," he said. "There is the possibility of the crush, and we know from similar incidents at sporting facilities and music festivals in the past that people can be irreversibly injured."

Dr Gannon said there were health and safety requirements for any business and that this incident raised questions about whether those precautions were observed.

"Sadly, there are many examples...where people have been crushed, sometimes crushed to death."

He said the organisers had a legal responsibility to provide a safe environment to the patrons including safe exits.

"[And] precaution against fire, facilities to go the toilet. for people to obtain safe, clean, drinking water; if it is a daytime festivals, shade and other shelter," Dr Gannon said.

"And they need to make arrangements for appropriately trained paramedics staff to have access to people who are injured or ill."

Dr Gannon said those remaining for the final night of the festival also needed to make sure they were safe.

Vivian Lees, founder of the Big Day Out, said of the organisers: "They're extremely experienced event producers and they have a very good safety record over many years and events that they've run so this is an exception for them."

Mr Lees said Falls Festival also attracted a younger demographic of audiences, who may be less experienced in managing their safety at festivals among large crowds.

He said that while management planned their programs based on the expected popularity of different acts, the promoters had here been "caught out", with such incidents escalating quickly.AJ Maddah, who ran rock festival Soundwave for more than a decade before it collapsed last year, said a combination of factors may have contributed to the stampede, including problems with the festival's program.The DMA's were scheduled to finish performing at the Grand Theatre tent at 9.50pm, the same time London Grammar was due to start at the Valley Stage stage at the opposite end of the festival."Sometimes, despite fans getting a little upset, it's good to have a few minutes overlap between one major act finishing and another starting to slow down the pace of the egress (or) similar acts on the same stage," he said. "If you have a situation where you have a bottleneck...you have to be super careful."Promoters could not always predict how many people would attend each act, and "no outdoor venue is perfect" he said. But crowd flows had to be staggered carefully, with security in place for major change-overs.

Mr Maddah said he had never experienced a similar incident while managing crowds larger than Falls of about 50,000 people and up to 95,000 in 2013.
