The Medical Board of Australia is now seeking to have general practitioner Hassan Alkazali's registration suspended "for a significant period of time", but not cancelled.

The Glenroy-based GP has been the subject of more than a dozen complaints over a decade, most relating to quality of care, and has been sanctioned at least five times but has continued seeing patients without conditions on his medical registration.

Dr Alkazali was last month found to have engaged in four counts of professional misconduct over inappropriate text messages to the female patient, and promises to help her get a pension for schizophrenia, a condition he could not have reasonably believed she suffered from.

The doctor had responded to questions about the status of the patient's pension application with: "But good dr needs good girl to play with."

In other messages he wrote: "Do you want to be mine," "U will get u form done perfectly good regardless u answer," "U will get pension for sure," "...I am asking if u want nice guy for casual sex very secretly," "Doesn't matter my dear ... I will use everything to get u pension."

The patient notified the Medical Board of Australia in 2013 but the case was not referred to the Victorian Civil and Administrative Appeals Tribunal until 2016.  A final decision from VCAT is expected in the coming weeks.

At a VCAT hearing on Wednesday, lawyers for the medical board revealed while the 2013 case was being investigated, a further complaint against Dr Alkazali was received and still under investigation.

Dr Alkazali had also been cautioned in relation to a third unrelated complaint in 2015, VCAT was told.

Christopher Winneke QC, representing the medical board, said Dr Alkazali had been the subject of "several complaints" related to his time practicing in Kinglake in 2007, five of which had resulted in the then Medical Practitioners Board of Victoria issuing either a caution, reprimand, period of mentoring, or further education.

The Medical Practitioners Board of Victoria was folded into the national Board of Australia when it was created in 2010.

On Wednesday, Mr Winneke said the Board had decided not to ask VCAT to consider all of the previous findings against Dr Alkazali when making its final determination, as most of the prior cases related to issues with his clinical competency.

VCAT vice president, Judge Pamela Jenkins disagreed and the details of five cases were read aloud, as well as a sixth case that had lead to a caution in 2015 to maintain professional boundaries after Dr Alkazali admitted contacting a patient by text message and providing them with free medication or money to obtain medication.

Dr Alkazali had also been cautioned in 2008 "not to let personal relationships with patients interfere with the quality of care he provides," when he was found guilty of unprofessional conduct for sub-optimal treatment of a male hypertension patient he had become friends with, Mr Winneke said.

Representing himself on Wednesday, Dr Alkazali said the Kinglake allegations were the result of a campaign by a disgruntled former landlord, who had rallied locals and the media against him.

He said he had undertaken further training and understood "how to respect the boundaries, how not to breach it in any way".

He described the text messages as "a significant mistake" and said he'd only intended them as "a joke".

He said the complaint that was being investigated was not sexual in nature.

"I'm a good citizen and my kids are good citizens and I didn't break the law," Dr Alkazali said.
