Portland police Sergeant Ray Hastings said four men from Ballarat visited another man at his property along the Woolsthorpe-Heywood Road at 12.15am.
Three men went inside the Heywood home, while one man stayed outside in his car.
That man went into the home a couple of times and eventually one of his Ballarat associates came out of the house armed with a hammer and struck the driver to the head.
The victim, a man in his 40s, was taken to the Portland Base Hospital for assessment suffering cuts and abrasions to his head and was later discharged.
His associates left the Heywood address in his car and headed back towards Ballarat.
When police arrived the man's car and his associates were gone.
Officers issued a lookout warning on the man's car and Hamilton police soon after attempted to intercept the vehicle.
The driver refused to pull over in Hamilton and evaded police.
Police investigations into the circumstances surrounding the incident are continuing.
