Ambulance Victoria Central Grampians operations group manager Graham McGrath said the man was in a serious condition when he was airlifted from the scene just before 1.30pm.
“He was trapped for some time by the fact the guard rail was pushed up against the car,” he said. “He is incredibly lucky, it could have been far more serious.”
Another woman was also taken to Ballarat Base Hospital after the accident which occurred just before midday.
The accident occurred at the intersection of Gillies and Clunes-Creswick roads.
