Police said the woman, 52, lost control of the car as she came around a bend on the highway five kilometres north of Clunes at about 1.30pm.
No other vehicles were involved in the incident.
Two ambulances, five police units, three CFA crews and five SES officers attended the scene.
The driver and two passengers in the car, a man, 41, and woman, 20, all from Talbot, were rescued from the wreckage by emergency crews.
Ballarat Highway Patrol Senior Constable Tony Creanor said all three were lucky to escape uninjured. He said the driver did not appear to be speeding and returned a blood alcohol reading of zero.
"We believe she has come around the bend and left the road suddenly and driven onto the soft surface on the side of the road which has caused her to lose control," he said.
Traffic from both sides of the highway remains blocked off as emergency crews work to clear the scene.
Senior Constable Creanor urged anybody who witnessed the incident to call Crime Stoppers 1800 333 000.
