The Ford sedan the 92-year-old man was driving hit a parked car on Beverin Street, Sebastopol, just before 4pm.
He sustained serious injuries and was initially transported to Ballarat Base Hospital before being airlifted.
Police are investigating the incident but no one else was hurt in the crash.
Highway Patrol Senior Sergeant Pat Cleary said police were notified of an erratic driver who was driving on the wrong side of the road in the area.
During the search for the vehicle, police were directed to the collision site between Vickers and Walker streets.
