Jayden Austin.
The 16-year-old was last seen leaving Ballarat Specialist School in Wendouree on August 16.
Police and family members are concerned for Austin’s welfare due to his age and he has an intellectual disability.
Investigators have released an image of Jayden in the hope that someone can provide information on his current whereabouts.
Jayden is about 170cm tall with a thin build, dark hair and brown eyes.
He is known to catch the train from Ballarat to Melbourne.
