One person has been taken to St John of God Hospital in Ballarat and one person has been taken to the Ballarat Base Hospital, according to an Ambulance Victoria spokesperson.
At least three vehicles were involved in the smash at Sulky that happened about 1.40pm.
Police say three cars were waiting at the roadworks sign, when a fourth crashed into them, causing a "concertina-effect".

The crash occurred near the overtaking lanes just before Creswick.
All emergency services were on scene.
The Midland Highway is blocked to traffic and people are asked to avoid the area.
