The honours up for grabs include Ballarat Sportsperson of the Year, Ballarat Junior Sportsperson of the Year (Wunhym Trophy - 17 or under during the calendar year), Mackenzie Award (administration), Widmer Award (athletes' mentors, coaches and trainers) and the Hollioake Medallion (team players).

Nominations require a full list of all results/criteria of the nominee and will only be considered if their principal place of residence or club is Ballarat.
