
Supporting the transition to Aged Care

Entering aged care can be an emotional experience for all involved.
Ballarat Aged Care has more than 150 years' experience in helping people transition to aged care, ensuring they feel supported, cared for and comfortable in their decision.
The Ballarat Aged Care admissions team members have assisted thousands of Ballarat families through the transition into aged care.
Admissions officer Amy Schilling said that more than 80 per cent of enquiries received by Ballarat Aged Care come from people who are looking to learn more about aged care for an older family member or loved one.
"Placing a loved one into aged care can be very stressful, and we approach every inquiry with understanding, compassion and care," Amy said.
"Our aim is to work with clients, their families and carer networks to provide the right information so they feel supported and confident in making an informed decision.
"The admissions office is the first point of contact for aged care enquiries, and we work with potential residents and their families from that first conversation through to the time of placement in their new home."
Ballarat Aged Care has 10 homes in the area, with a range of options depending on the resident's location, needs and level of independence.
As a Centre of Excellence in Aged Care, residents have access to registered nurses, specialised care from geriatricians, palliative care and rehabilitation consultants, and nurse practitioners.
The admissions team is available at each step of the process, from discussing each of the 10 homes, helping to understand the fees and available government support, to assisting with filling out documentation and setting up any required payments.
"We know that there's no "one-size-fits-all" solution to aged care, so we work to ensure that every person entering care finds a home where they feel comfortable, safe, cared for and engaged," Amy said.
"Each of the Ballarat Aged Care homes has a real sense of community, which is so important to residents and their families.
"We often have residents' loved ones contact us following a placement to let us know how grateful they are knowing that their loved one is happy in their new home.
"It's really satisfying to know that the support we offer can make the transition to aged care more positive and less stressful."
Ballarat Aged Care is part of Ballarat Health Services, and has been providing professional, quality aged care services to the Ballarat and Grampians regions for more than 150 years.
There are 10 aged care homes available throughout the region, catering for residential aged care, dementia and cognitive impairment care, psychogeriatric care, respite and in-home care.
