It comes on a day when Victoria registered a dip in new cases, with 275 further instances of the infection reported overnight.
Ballarat has now seen a total of 20 cases since the epidemic began, with nine of them now active.
So far, there are no further details on the location of the extra individual with the virus, where they may have picked the virus up and what their condition is.
Golden Plains also registered one additional case of the virus on Monday. There is now a total of five active cases in Golden Plains and three in Moorabool Shire.
The cluster in Colac-Otway - linked to a meatworks in Colac - continues to expand with a total of 12 cases now linked to that location.
Ballarat Health Services had confirmed on Saturday that one of the residents at the Bill Crawford Lodge had tested positive.
It followed the positive test of a staff member earlier in the week, whom a BHS spokesperson said had behaved "cautiously and responsibly" and had self isolated.
In a statement, BHS said:
"The resident has been in isolation since flu-like symptoms appeared and they were tested as soon as these symptoms presented as per current COVID-19 precautions.
"The resident has been transferred to the Ballarat Base Hospital and is in a stable condition.
"The staff member has not needed to come to hospital.
"Staff and visitors who are identified as being a close contact of these positive cases have been contacted and immediately tested and will be followed up over the next 14 days, regardless of symptoms.
"All other people who have visited or worked at Bill Crawford Lodge need to be vigilant for any symptoms, then get tested as soon as symptoms appear."
On Thursday last week, the first active case in two months was reported in the Ballarat local government area. The previous positive case dated back to May 17.
However, there were four more announced in Ballarat on Friday and three more announced on Saturday.
The tally has been increasing slowly but significantly in regional areas.
Two schools have closed in the Geelong area, while remote learning begins again for most of the students in lockdown areas of Melbourne and Mitchell Shire.
On Saturday, a new testing clinic was announced at the Ballarat Seniors Centre on Little Bridge Street.
