The three-time premiership player departs the Hawks under unrestricted free agency as a three-time premiership player.
Smith, 31, has accepted a two-year deal with Geelong.
He has played 210 games in 10 years with Hawthorn, including the 2013, 2014 and 2015 premierships.
Smith was one of the big stories of the 2010 AFL national draft, being picked up by the Hawks with selection 19 after just a half-season with the Roosters as a member of their first VFL premiership side..
Originally from Cootamundra in NSW, Smith moved to Ballarat to attend university - joining Redan in the Ballarat Football League to have a kick with mates.
He quickly impressed, playing in Redan's 2009 premiership team and then being persuaded to join North Ballarat mid-way through the next season.
