Jay Lee, of Delacombe, was sentenced at the Ballarat Magistrates' Court on Friday after he pleaded guilty to 13 offences.
On September 2 and 5, Lee contacted his ex-girlfriend's family and friends wanting to know her location. He threatened to kill her friends and family if they did not tell him.
The father-of-one told his ex-partner's sister he was outside her house and threatened to ram his car through, slit her and her children's throats and drag them out of the house if she did not provide the location.
He called his ex-girlfriend's mother and said he would drive to her house and kill her and any grandchildren with her. He was arrested on September 5 and charged with making threats to kill.
Five months before the threats on April 25, Lee and his ex-girlfriend became involved in an argument and Lee smashed his neighbour's window causing glass to scatter over sleeping children.
Magistrate Letizia Torres told Lee on Friday she hoped he understood the seriousness of his offending now that he was not using drugs and had a clear head.
"Your behaviour was very dangerous and would have been extremely frightening for the multiple victims involved," Ms Torres said.
"You were out of control and have similar priors for this type of offending."
The magistrate said she took into account Lee had personality and conduct disorders, which had affected his education, but drug use had made everything far worse.

She said Lee's sentence would be reduced because he was young and it was important he lived a productive life.
Lee was sentenced to four months' imprisonment followed by a 12-month community corrections order which includes supervision.
Ms Torres said the order would not include specific programs because they were available through Lee's service providers, including the National Disability Insurance Scheme.
The 54 days Lee has already spent in custody will count towards his sentence.
