It was a satisfying win for the club, which narrowly fell to City in the preliminary final last season. Midlands skipper David Speechley said he could not have asked for a better start to the 2020-21 campaign.
"It feels really good, they're the benchmark and they've always been a competitive side in Midweek and Saturday Pennant," he said.
"For us to beat them gives us a real boost."
Midlands won three out three rinks to secure the match 70-58 at City Oval on Tuesday. Speechley enjoyed a four-shot win over Jenny Harman, while Paul Carlyle edged out Wayne Roberts by three shots.
With Midlands up four shots with three ends to play, Paul Kennedy had his work cut out to keep Ian Robinson at bay. He succeeded, securing a five on the second last end to put the match out of reach.
"To get a five on the second last end to save the match was great," Speechley said.
"We were down early in the first few rinks but from five ends on we were in front and held out most of the way."
Meanwhile Creswick has overcome Sebastopol by 27 shots at Sebastopol. Gerry Flapper comprehensively beat Ian Hedger by 24 shots, while Alan Annear beat Maxwell Medwell. Sebastopol's David Pratt edged out Beth Huntley.
Elsewhere Central Wendouree beat Clunes and BMS beat Bacchus Marsh.
