Police believe the blue Holden had been travelling along Pitfield-Scarsdale Road when the driver veered across the road and crashed into a tree about 8pm.
The car then rolled and hit another tree before coming to a stop.
Emergency services were notified of the single-vehicle crash near the corner of Pitfield-Scarsdale Rd and Golden Lake Rd at Newtown just before 8.30pm.
The car was located several metres down an embankment, with the driver trapped in the car by a large tree branch.
Firefighters worked to free the woman, who had sustained multiple injuries.
Country Fire Authority crews had six vehicles at the scene - from Smythesdale and Ballarat Fire Brigade - and were supported by Fire Rescue Victoria.
The driver and sole occupant of the car, a 24-year-old Wendouree woman, was freed after about an hour later and flown by air ambulance to the Royal Melbourne Hospital in a serious but stable condition.
Ballarat Highway Patrol are investigating the circumstances surrounding the collision.
