Ballarat Health Services announced the change after the state government expanded eligibility to younger people on Sunday.
Bookings must be made ahead of time, and informed consent must be given before the vaccination.
In a media statement, BHS said it was a "welcome development".
"To assist people in providing informed consent, Ballarat Health Services will have a doctor working on site along with experienced nurse immunisers, to answer questions," the release states.
"Anyone aged 18-39 seeking to protect themselves from COVID-19 with the AstraZeneca vaccine must make an appointment via the state booking system.
"Anyone eligible for the COVID-19 vaccine can make an appointment via the Victorian Government's Coronavirus Hotline on 1800 675 398 or via the online portal https://portal.cvms.vic.gov.au/.
"Walk-in appointments are only available for members of our community aged 60 years and over for the AstraZeneca vaccine.

"Committee for Ballarat and Ballarat City Council are also asking our community to get vaccinated as soon as possible, with important messages from our community leaders: Roll up your sleeve and get vaccinated. We're in this together, Ballarat."
Committee for Ballarat chief executive Michael Poulton said it was "fantastic" news.
"There are 700 appointments available at the Mercure every day, and we want every one of those filled every day," he said.
"The more we fill them, the more demand grows, the more supply we get.
"I reckon (state Chief Health Officer) Brett Sutton's comments yesterday, the best vaccine for you is ithe one you can get now - what it says today is you can get one now with informed consent."
"For 18 to 39-year-olds, here is your opportunity to contribute to our COVID recovery, because that's the only way we'll get out of this in the long term."
He also urged anyone else eligible to book in as soon as they could.
The AstraZeneca vaccine is also available to young people through GPs.
There were fears last week that of a vaccine shortage, with BHS forced to limit walkups to the vaccine hub.
