She takes over from interim operations lead Damian Ross, who stepped in mid-season.
He decided not to apply for the role in the long-term.
Brown said it was an honour to return to the place where her elite talent pathway journey began.
"It's exciting to be able to come back to the GWV Rebels - a place that gave me my first opportunity within an elite pathways program (as a statistician, team manager and welfare manager)," she said.
"To be able to pay it forward and give back to the region is something that I am extremely excited about and will take great pride in.
"I am excited to continue to give opportunities to both boys and girls throughout the region and ensure that they are able to grow as not just a footballer but also a valuable member of their local communities."
NAB League manager for Country Lee Fraser said Brown was the clear choice.
"Brooke has demonstrated the attributes we look for and shows an unwavering commitment to the NAB League boys and girls programs," he said.
"Brooke is primed to take the GWV Rebels region forward. She understands the history of the region and its place in the Victorian Talent Pathway as a significant contributor to building better footballers and better people."
Along with her previous roles at the Rebels, Brown also led Lake Wendouree's under 10 side in 2016, the first female footy coach in country Victoria.
