Watchdog tells Metro Trains to fix serious safety flaws in wake of teen's death

The rail safety watchdog has told Metro Trains to fix serious safety flaws that contributed to the death of an 18-year-old man who fell between a moving train and platform in Toorak two years ago, or risk further casualties on Melbourne's railways.
