It is believed a south-bound truck and a north-bound car collided.
The truck driver has been taken to Ballarat Base Hospital in a stable condition.
Police have also established a crime scene as they investigate the crash.
Motorists are urged to avoid the area.
Sergeant Michael Kearns said the head-on collision occurred just before 3pm.
“The head-on collision occurred just prior to 3pm between a north-bound sedan and south-bound truck on Ballan-Daylesford Road in Ballan,” Sergeant Kearns said.
“The driver of the motor vehicle is deceased.
“(The) Driver of truck has sustained injuries and taken to hospital (in an ambulance) in a stable condition and was breathing when he left the scene.
“Police are pursuing number of lines of inquiry as cause to accident.”
3.30PM: Ballan-Daylesford Road has now been blocked off in both directions.
Police have also established a crime scene as they investigate the crash.
3.10PM: All emergency services are currently attending a serious traffic accident in Ballan.
The collision, believed to be between a car and a truck, occurred on Ballan-Daylesford Road, just south of Dehnerts Road.
There is expected to be a number of traffic diversions in place for some time and motorists should avoid the area.
