Mark Humphrey, 30, was last seen leaving his home address on Riversdale Crescent on Saturday 19 November about 10pm.
Police and his family have concerns for his welfare as he has a medical condition.
Police have released an image of Mark in the hope someone recognises him and can provide information on his current whereabouts.
He is described as 171cm tall, with a medium build and has shaved stripes along both sides of his head.
Mark was last seen wearing a dark-coloured Fox hooded jumper and grey or black tracksuit pants.
Police have been told Mark is known to frequent the Bacchus Marsh, Darley and Melton areas.
Anyone with information about Mark is urged to contact Bacchus Marsh Police Station on 5366 4500.
