Victoria Police issued a statement advising that Major Collision Investigation Unit detectives have charged the man, who is under police guard in hospital.
Police say a Ford Telstar and a Toyota Hilux collided at the intersection of the Old Glenelg Highway and Glenelg Highway about 5pm last Friday.
A 17-year-old Scarsdale woman passenger in the Telstar died at the location of the accident.
An air ambulance was called in and the driver of the Telstar was airlifted to a Melbourne hospital with serious but non-life threatening injuries.
He remains in hospital with spinal and other injuries..
The three occupants of the Toyota Hilux – a 25-year-old Scarsdale man who was driving, a 57-year-old Queensland man and a 50-year-old Scarsdale woman – were all taken to a local hospital for observation.
Detectives have charged the 34-year-old Telstar driver with dangerous driving causing death, evading police, failing to obey a Give Way sign and unlicensed driving.
He has been bailed to appear at Ballarat Magistrates’ Court on January 11.
Ballarat Police have again urged drivers to be cautious and to drive sensibly.
Senior Sergeant Damian Ferrari warned that people must be fully aware of what is taking place around them when driving and never be complacent.
“I think it is just a timely reminder of how dangerous it can be on the roads and for everyone to take care, drive carefully and get to their destination safe,” Senior Sergeant Ferrari said.
The Victorian road toll for 2017 is four, two more than at the same time last year.
