Police first tried to intercept a stolen white Holden ute on Grant Street in Sebastopol just after midnight.
But the ute failed to stop and a police helicopter was called in to track the vehicle as patrol cars dropped back to a safe distance.
While details remain unclear, officers said the offenders tried to carjack a vehicle on Remembrance Drive in Alfredton about 4am.
Police activated their lights and sirens and started to pursue the offenders before their car lost control and veered off the road, smashing into a fence and property on Webb Road in Bonshaw.
A 28-year-old Sebastopol man was taken to hospital with minor injuries.
A 17-year-old boy and a 24-year-old woman, both from Ballarat, have been taken into police custody.
