Although Dziwoki is still a Melbourne Vixens player for the upcoming ANZ Championship season, she will be free to play for the Sovereigns when her commitments allow.
She has nominated Ballarat as her home club, which operates under the same system as the Victorian Football League.
When available, the 186-centimetre goaler will form a formidable duo with former Australian shooter and current Springbank coach, Cynna Kydd, in attack for Ballarat.
It is expected Dziwoki will be able to play the majority of games for Ballarat, bar the occasional clash when the Sovereigns play on the weekend.
Sovereigns head coach Natasha Chokljat said it was a great signing for the club, which would bring it in line with all other VNL clubs in terms of elite players on their list.
Dziwoki will mainly play in attack, but could also be thrown down the defensive end depending on the Vixens' wishes.
"It's fantastic for the club, we've got a real mix of players now," Chokljat said.
"Playing with those experienced players is great, the young girls will learn so much from having someone like Carla along and hopefully it can lead to developing our own national league netballers."
Dziwoki, an Australian representative herself, only signed with the Vixens in August, having spent the previous four seasons with Sydney Swifts in Australia's premier netball competition.
The 32-year-old has had a decorated career that includes being named in the Australian squad in 2004, as well as representing the nation in the Fast5 World Netball Series in 2011 and as recently as 2013.
Ballarat's newly formed side, which was granted a VNL licence after the struggling Ballarat Pride was axed earlier this year, will be coached by former Australian Diamonds player Natasha Chokljat, with former Australian teammate Eloise Southby to fill the role as high performance director.
The Sovereigns' first game will be on February 7, with the season brought forward due to the Netball World Cup in August.
