Rampe was in the middle of a planned rehabilitation run after the round one loss to Port Adelaide when he tripped over a chain and landed on his arm, breaking it.
Coach John Longmire confirmed the circumstances of the injury when addressing the media on Monday.
"Rampe went for a run this morning and jumped over a chain on the run, landed on his arm and broke it."
The All-Australian defender has been a constant in the Swans back six since making his debut in 2013.
The club was in the middle of preparations to fly relatives to Melbourne for Friday night's clash with the Western Bulldogs; which would have been Rampe's 100th game.
"I couldn't believe it to be honest, to fall on it trying to jump the chain then falling on the concrete 20 metres from the front door," Longmire said.
"He has a crack just below his elbow and at this stage I have been told it is a minimum of five weeks.
"I went through the game with him an hour beforehand and so obviously it is very disappointing."
After making his debut in the opening round of 2013 against Greater Western Sydney, Rampe was then dropped and named as emergency for the next two weeks.
He broke back into the team in round four and hasn't missed a game since, playing 98 consecutive games.
Aliir Aliir, who played in the NEAFL on the weekend, will lead a pack of several candidates to take Rampe's place in the Swans defence.
