Shane Crewdson (also known as Shane Evans) was apprehended by Ballarat police about 11.45am at a residence in Mount Pleasant after a short chase.
Police say he tried to escape on foot when officers arrived, but he was apprehended a short time later.
Police have hit him with 27 charges, including five counts of allegedly contravening a family violence order with intention to cause harm, four counts of alleged assault, alleged dangerous driving and two counts of alleged criminal damage resulting from incidents on December 3.
He is expected to appear in Ballarat Magistrates Court on Tuesday.
