Two cars collided on the Western Freeway just after 9.30pm in Wallace.
A spokesperson from Brimbank Highway Patrol confirmed to The Courier on Thursday morning that one car was travelling on the wrong side of the road.
The driver and only occupant of the one of the cars has died at the scene and is yet to be formally identified.
The two occupants of the second car have been taken to hospital.
A woman aged in her 40s has pelvis and leg injuries. She was taken by road to Royal Melbourne Hospital in a stable condition.
A man aged in his 50s has chest injuries. He was treated by paramedics and taken to Ballarat Base Hospital in a stable condition.
The exact cause of the collision remains unclear and is being investigated.
It is believed one of the cars involved was a taxi.

EARLIER
Emergency crews rushed to a serious head-on crash on the Western Freeway last night.
There were two cars involved in the smash in between Wallace and Gordon, which occurred about 9.40pm.
An air ambulance which arrived at the crash at 10.55pm has remained on the scene for at least half an hour, with a number of CFA tankers and police around the crash site.
CFA taper has been strung around the site of impact, which stretches across the median strip and the west-bound lanes.
The Ballarat-bound lanes of the Western Freeway were blocked to traffic.
Injuries are not yet known.
People are advised to avoid the area.
Lisa Volwes said online she had been stuck in the traffic for more than an hour, with her “thoughts and prayers” going to those injured in the crash.
This is the latest in a series of crashes in the Ballarat area, particular to the city’s east.
A motorcyclist was killed near Ballan on January 18, a young woman was killed when a four-wheel drive flipped on January 24 and another young woman lost her life when her car left the road near Mt Mercer on January 25.
