Ballarat investigators have been told a 54-year-old man was driving a white utility south along Wiltshire Lane on Thursday, May 13.
It was about 6.35pm when another vehicle pulled out in front of him.
The man, from Wendouree, overtook the vehicle - believed to be a 2010 silver Ford Falcon sedan - before continuing on.
The unknown driver followed him to a carpark on the Glenelg Highway.
Both men then became involved in a verbal altercation, before the unknown man struck the victim several times to the upper body, causing him to fall and hit his head on the ground.
The offender then returned to his vehicle and drove away - leaving the victim with serious head, facial and hand injuries.
It is not known which direction the offender drove off in.
Investigators have released CCTV footage of a Ford Falcon sedan and a man who they believe can assist with their enquiries.
The man is perceived to be Caucasian and aged between 20 and 25 years. He has an athletic build, brown hair and is approximately 193cm tall.
He was wearing a black long-sleeve jumper, black shorts and was barefoot at the time of the incident.
Anyone who recognises the man or the vehicle is urged to contact Crime Stoppers on 1800 333 000 or file a confidential report online at www.crimestoppersvic.com.au
