The man had been driving a black Holden Commodore sedan with multiple passengers when he allegedly collided with a grey Mazda sedan about 11.50am.
All emergency services, including police, firefighters and paramedics, rushed to the scene.
Emergency services worked on the driver and sole occupant of the Mazda, a 71-year-old Alfredton woman, for about an hour.
While an air ambulance arrived near the scene, the woman sadly died before she could be transported.

The man was remanded in custody and will appear before the Ballarat Magistrates' Court on Monday May, 24.
